Ok here is the situation.
I was playing against AI and I bombed an enemy planet killing off all there colonists and got the bombing report.
Ok but here is the strange part.
I had no bombers on that planet any more and there was clearly no battle report. I checked twice.
Is this error even possible? Just disappearing seems far fetched.

Maybe they scrspped themselves?

Have you played the next turn after that? If not then you could backup & double check what was supposed to happen.

I remember reading somewhere that there is a limit for the number of ships you can have in a same fleet (1022?). If you go over that number then some ships will disappear. Ships with the highest ID number will disappear first.
Edit: I've also just found this in the Wiki.

The limit for how many ships of the same design you can have in a fleet is 32767. I think he'd have mentioned it if there were *that* many bombers.
Possibly deleted though.

You probably deleted the old frigate design after the battle & designed a new B52 in that turn. If you did that then the battle would show the new ship design & icon but the battle result correctly treats the ships as the old frigates.
A similar thing happens if you scrap some ships or deleted a slot & used the same slot to design something. If you delete a slot & don't reuse it you will see a message re. the scapping that does not specify the ship name.

Welcome to my world. I've had the battle board show me a different outcome than the actual battle, though reinstalling fixed that so I have to assume the .exe got corrupted somehow in that case. I'm thinking of reinstalling again since this is the first time I've had this problem.

No, normal game with no special changes. One thing of note, the transports were never alone in the system of origin the year they moved. Maybe it was a mixup with fleet numbers in the .x file.
I do a lot of merging and splitting of fleets, and generally at the start of a turn if there are transports in a system that have no orders I will merge them. Reduces clutter while I'm looking at other things and I can quickly tell how much transport capacity I have available. If some orders, like say the split, manual load/transfer, move and wp1 unload came after a previous "save and submit" then maybe they didn't get recognized.
Like I said before I delete the .x file while the game is open before doing another save, but maybe the submit part of it got loaded in as well? Normally I just save and only save and submit when I'm done with the turn; but if the deadline is close I'll do save and submits just so I can do a quick submission if need be. I guess in the future I'm going to have to do just saves even in that situation.

Had any of your planets perhaps "Route"-orders?
Automatic planet orders to route ships to another planet. Wether those are on or off can be seen here... see link for picture:

For gating those can be quite tricky because they work only for ships within the weight- and distance gate limits. If those are met, ships are gated, if not they are sent thru space (I think... not sure wether possibly nothing happens or a message gets displayed a year later)
