When do the various types of scanning take place? All of the sources of these scans can disappear in a turn, so it matters. I suspect (but am not sure) if they exist at the beginning of a turn (or built/grow during the turn), they get the scan for where they are at the end. You won't see the yellow and red circles, but the information will be there.
* Ship normal & pen scanning
* Remote miners non-moving mining scanning of planets
* AR starbase pop scanning
* IT gate scanning
* PP packet scanning

If a ship is destroyed in battle it only gets the number, design and location of the fleet(s) that destroy it, and nothing else, not even the direction these ships came from.

This however is not the result of scanning proper(as in scanning fields red or green). It is much like information on fleets in orbit, obtained at a planet without a scanner.
